In partnership with
Omega Financial, Inc., there are now three ways to submit your membership
dues. If you have any trouble submitting your dues on-line, please e-mail Victor Felts.
Please choose one of the following:

Complete the application form electronically, print it, and mail it with a check for dues.

Complete the application
form on-line and mail your check for dues separately. (If
you mail dues separately, they must be postmarked by November
15, 2007, to avoid the late fee.)

Complete the application form on-line and pay your dues on-line with a credit card or electronic check.
Add a late fee of $50 if membership dues are paid after November 15,
2007.
